Why Can a Stroke Occur Despite Anticoagulation?

Quick answer: A stroke can occur because anticoagulation was interrupted or inadequate, or because the blockage arose through a different disease mechanism.

Anticoagulants substantially reduce atrial fibrillation-related stroke risk, but they cannot prevent every ischemic stroke. What Tests Are Needed After a Stroke on Blood Thinners?

Quick answer: Evaluation combines urgent brain and vascular imaging with a detailed review of anticoagulant use and possible alternative sources of embolism.

Emergency assessment begins with CT or MRI to distinguish ischemic stroke from bleeding and to define the affected brain tissue. Vascular imaging can identify a blocked artery, significant plaque, or another arterial disorder. Depending on the findings, clinicians may use electrocardiographic monitoring and echocardiography to investigate cardiac sources of embolism, including thrombus or structural heart disease.

The medication review is equally important. Clinicians check the prescribed dose, refill history, recent interruptions, kidney and liver function, and medicines or supplements that could interact with treatment. For people taking warfarin, current and previous international normalized ratio measurements help show whether anticoagulation has been consistently therapeutic. Routine clotting tests cannot reliably quantify the effect of every direct oral anticoagulant, so results must be interpreted carefully.

Should Anticoagulants Be Switched After a Breakthrough Stroke?

Quick answer: Changing anticoagulants may be appropriate in selected cases, but an automatic switch is not supported without first identifying the likely cause.

Treatment should be individualized according to the stroke mechanism and the reason protection was inadequate. Correcting an unsuitable dose, improving adherence, addressing an interaction, or treating symptomatic arterial disease may be more relevant than choosing a different anticoagulant. The timing of restarting treatment also depends on infarct size, bleeding risk, brain imaging, and the patient's clinical condition.

Adding an antiplatelet drug such as aspirin to anticoagulation is not a routine solution because combined therapy generally increases bleeding risk and should have a separate clinical indication. Procedures involving the left atrial appendage may be considered for carefully selected patients, particularly when long-term anticoagulation is contraindicated, but they are not a universal response to one breakthrough event. Decisions are best made jointly by stroke and cardiovascular specialists.
