Interpretation
- 13–15: Mild brain injury / minimal impairment
- 9–12: Moderate brain injury
- 3–8: Severe brain injury (coma); ≤8 generally requires intubation

Always document E/V/M breakdown. The same total can reflect different clinical pictures — E1V2M5 is very different from E3V3M2.

FOUR (Full Outline of UnResponsiveness) is an alternative for intubated patients where verbal can't be assessed. Use whichever your institution standardizes.
